Sagebrush Recovery provides outpatient treatment and recovery support for adults experiencing substance use and co-occurring mental health concerns.
Our integrated approach brings together clinical care, peer support, practical skill-building, and community connection—allowing each person’s services to reflect their needs, goals, and stage of recovery.
Services are available through telehealth and, depending on the service and location, in person throughout Colorado.
Intensive Outpatient Program (IOP)
More structure and support without stepping away from everyday life.
Sagebrush Recovery’s Intensive Outpatient Program (IOP) is designed for adults who need a higher level of care than traditional outpatient treatment but do not require residential or inpatient services.
Our virtual evening program combines structured clinical treatment with individualized support while allowing participants to continue working, caring for family, and remaining connected to their communities.

Our clinical team provides outpatient treatment for adults seeking support with substance use, recovery, mental health concerns, and the challenges that often accompany major life change.
Treatment is individualized rather than one-size-fits-all. Depending on clinical need and treatment goals, services may include individual counseling, therapy, group treatment, assessment, and ongoing treatment planning.

Clinical Groups

Outpatient Clinical Care
Support from people who understand that recovery happens outside the therapy room, too.
Peer support is an important part of Sagebrush Recovery’s integrated model.
Our peer and behavioral health support team provides non-clinical, person-centered support focused on helping individuals identify their own goals, strengthen recovery capital, navigate challenges, and build meaningful lives in recovery.
Support may include:
Developing recovery goals and routines
Identifying strengths, barriers, and next steps
Building accountability and healthy structure
Navigating difficult moments and recovery challenges
Connecting with community resources and recovery supports
Exploring employment, education, wellness, and life goals
Strengthening natural supports and community connection
Reentry support for justice-involved individuals
Peer services may be provided virtually, by phone, or in person when available in the client’s community.

Sagebrush offers supportive programming that helps clients build practical skills, confidence, connection, and independence.

Some groups are clinically facilitated while others are led by qualified peer or behavioral health support staff. Available groups may vary by location, population, and current programming.

Groups, Life Skills & Community-Based Support
Where We Provide Care
Colorado care with roots in mountain communities.
Clinical services are available through telehealth to eligible clients throughout Colorado.
Sagebrush also provides in-person peer and community-based services in select communities across Colorado’s mountain regions, including areas along the Front Range, Central Mountains, and Western Slope.
Our home office is located in Salida, Colorado, and we continue to expand access in communities where behavioral health and recovery resources can be difficult to reach.
Insurance & Payment
Coverage depends on the service being provided, the client’s health plan, geographic region, provider eligibility, and current payer agreements.
Sagebrush works with Colorado Medicaid members and also offers self-pay options for certain services. Because coverage can differ between programs and services, our team will help determine eligibility and available options before services begin.
