Sagebrush Recovery partners with healthcare providers, behavioral health organizations, hospitals, justice-system professionals, case managers, and community agencies throughout Colorado to help adults access substance use and co-occurring mental health treatment.

Our integrated model combines clinical care, peer and recovery support, practical skill-building, and community connection—giving clients access to individualized services while supporting continuity across systems of care.

Integrated Clinical + Recovery Support


Telehealth clinical services help us reach clients throughout Colorado, while in-person peer and community-based services are available in select communities. Our model is especially well suited to rural and mountain regions where behavioral health resources may be limited.

We value communication and coordination with referral partners and, when appropriate authorization is in place, work collaboratively to support continuity of care.


Care is based on each client’s needs, goals, strengths, clinical presentation, and appropriate level of care rather than a one-size-fits-all program.

Our team works with clients navigating challenges that may include justice involvement, reentry, housing instability, employment barriers, limited transportation, geographic isolation, and other factors that can interfere with treatment and recovery.

Intensive Outpatient Program (IOP)
Structured clinical treatment for adults needing more support than traditional outpatient care while remaining in their homes and communities. Contact our team for current enrollment availability.

Peer & Recovery Support
Individualized, non-clinical support focused on recovery goals, strengths, accountability, community connection, and building recovery capital.

Outpatient Clinical Care
Individual counseling and therapy, assessment, treatment planning, clinical groups, and support for substance use and co-occurring mental health concerns.

Groups & Life Skills
Clinical and behavioral health programming focused on coping skills, emotional regulation, relapse prevention, employment, daily living skills, wellness, and other practical areas of recovery.

Who We Serve

Sagebrush Recovery serves adults who may benefit from support related to:

  • Substance use and recovery

  • Co-occurring mental health concerns

  • Relapse prevention and recovery maintenance

  • Transition from residential, inpatient, detoxification, or other higher levels of care

  • Justice involvement, reentry, probation, parole, or community corrections

  • Difficulty engaging in or maintaining traditional treatment

  • Rural or geographic barriers to behavioral health care

  • Social and practical barriers that may affect treatment or recovery

We Believe Good Care Is Collaborative

Sagebrush recognizes that clients often receive support from multiple providers, agencies, and systems. With appropriate client authorization, we welcome collaboration with members of the client’s care and support network.

Depending on the client’s needs and applicable permissions, coordination may include communication regarding:

  • Referral and enrollment status

  • Participation in services

  • Treatment and recovery needs

  • Transitions between levels of care

  • Discharge and continuity planning

  • Community resources and additional supports

Our goal is to complement—not duplicate—the work already being done by the professionals and organizations supporting each client.
