Our trauma-informed, whole-person approach brings together clinical treatment, lived experience, and connection.
Registration is open. The program begins September 28, 2026, with rolling admissions after launch.
Your weekly schedule
- Group treatment: Monday, Tuesday, and Thursday, 5:30–8:30 p.m. Mountain Time.
- Individual therapy: One session each week.
- Time commitment: 10 hours per week.
- Program length: 12 weeks.
- Location: Online for adults throughout Colorado.
The rolling curriculum allows eligible clients to join after the program launches without waiting for a new cohort.
Is virtual IOP right for you?
Our IOP is an ASAM Level 2.1 substance use disorder program. It may be appropriate if you are stepping down from residential treatment or need more structure than traditional outpatient care and can safely participate while living in the community.
A clinical assessment helps determine whether IOP matches your treatment needs, health, and current stability. Co-occurring mental health concerns can be addressed alongside substance use treatment when outpatient care is appropriate. A substance use treatment need is required for IOP; adults seeking mental health therapy alone can explore our outpatient services.
Sagebrush does not provide inpatient medical detoxification, 24-hour psychiatric stabilization, or continuous medical monitoring. If your needs require a higher level of care, our team will help identify an appropriate next step.
What treatment includes
Clinical groups and weekly individual therapy support recovery skills, coping, emotional regulation, and relapse prevention. Your care begins with an assessment and an individualized treatment plan. Peer recovery support and community resources may complement clinical treatment based on your needs and plan.
Our approach is rooted in lived experience and community connection. Virtual care helps adults in rural Colorado participate without traveling to a treatment center three evenings each week.
Colorado Medicaid and payment options
Sagebrush accepts Colorado Medicaid and offers private-pay options. Before treatment begins, our team will help verify eligibility, benefits, and any applicable authorization requirements. Coverage depends on your plan and the services provided. Contact us about other insurance options.

Questions about virtual IOP
Do I need to live near Salida?
No. Our IOP is delivered virtually to eligible adults throughout Colorado. In-person peer support is available separately in select communities.
Can I join after September 28?
Yes. After launch, the rolling curriculum allows eligible clients to join throughout the program cycle. Your start date is arranged after assessment and admission.
Is this a mental health IOP?
This is a substance use disorder IOP that can also address co-occurring mental health concerns. Individual outpatient mental health therapy is available for adults who do not have a substance use diagnosis.
Can I participate while working or caring for family?
Evening group sessions are designed to fit alongside daily responsibilities. Plan for three three-hour evening groups and one individual therapy session each week.
How do I know whether I need IOP or residential treatment?
Our clinical assessment helps determine the appropriate level of care. Contact us to discuss your needs rather than trying to make that decision on your own.
